Please print, fill out, and bring the following forms to our first appointment:

Fees
The fee for a 50 minute session is $80. The fee for an 80 minute session is $120. Payments must be made at the beginning of each session by cash or check. Full payment is required for sessions cancelled with less than 24 hours notice.
